Come Prepared. Stay Compliant.

Bringing the right documents and information helps your appointment move quickly and avoids the need for a follow-up visit.
  • Government-issued photo ID (driver’s license, passport, or state ID)
  • Current Medical Examiner’s Certificate (if you’re renewing)
  • Complete list of current medications, including dosages
  • Glasses, contact lenses, or hearing aids you currently use
  • Documentation from your treating physician for any chronic conditions
  • Recent test results or specialist letters for managed conditions (diabetes, sleep apnea, heart conditions)
  • Payment for the exam

Note: If your vaccination records are incomplete, bring whatever you have. We’ll walk you through your options.

Your Medical Examiner's Certificate is valid for up to 24 months. Drivers managing certain conditions, such as hypertension or sleep apnea, may receive a 12-month certificate that requires earlier renewal. We recommend scheduling your next exam before your current card expires to keep your CDL active.

Transparent Pricing. No Surprises.

The DOT physical exam fee covers your complete medical evaluation and your Medical Examiner’s Certificate. We do not bill health insurance for DOT physicals. Payment is due at the time of service.
