What Causes a Sore Throat?
Viral infections are the most common culprit. Colds, the flu, COVID-19, and other respiratory viruses often cause throat pain along with symptoms like congestion, cough, and fatigue. These infections typically improve on their own within a week or so, though symptom management can help you feel more comfortable during recovery.
Bacterial infections, particularly strep throat caused by group A Streptococcus bacteria, require a different approach. Strep throat won’t go away on its own and needs antibiotic treatment to clear the infection and prevent serious complications.
Other causes of sore throat include allergies and postnasal drip, dry air or mouth breathing during sleep, acid reflux, environmental irritants like smoke or pollution, and voice strain from excessive talking or yelling.
Strep Throat vs. Viral Sore Throat
Strep throat often comes on suddenly with severe throat pain, pain when swallowing, fever, red and swollen tonsils sometimes with white patches, tiny red spots on the roof of the mouth, and swollen lymph nodes in the neck. Notably, strep throat usually does not cause cough, runny nose, or congestion.
Viral sore throats tend to develop more gradually and are often accompanied by other cold symptoms like sneezing, coughing, congestion, and watery eyes. Fever may be present but is often lower than with strep.
Because symptoms overlap, testing is the only reliable way to confirm whether strep bacteria are responsible for your sore throat.
Why Strep Testing Matters
If you do have strep, antibiotics will help you feel better faster and reduce how long you’re contagious. More importantly, untreated strep can lead to serious complications including rheumatic fever, which can damage the heart, and kidney inflammation. These complications are rare but preventable with proper treatment.
If your sore throat is viral, antibiotics won’t help and could contribute to antibiotic resistance. Knowing for certain what’s causing your symptoms helps ensure you get appropriate care.
What to Expect During Your Visit
Your provider will examine your throat, tonsils, and lymph nodes. If strep is suspected, we’ll perform a rapid strep test, which involves a quick swab of the back of your throat. Results are available within minutes, allowing us to make a diagnosis and start treatment during the same visit if needed.
In some cases, if the rapid test is negative but strep is still strongly suspected, we may recommend a throat culture for confirmation. We’ll discuss the best approach based on your individual situation.

Most people start feeling significantly better within a day or two of starting antibiotics. You’re generally considered no longer contagious after 24 hours on medication, which means you can return to work or school sooner.
