Do You Need an Immigration Medical Exam?

This includes:

Family-based green card applicants

Employment-based green card applicants

Diversity visa lottery winners adjusting status

Certain other categories as specified by USCIS

Not sure if this applies to you? Your immigration attorney can confirm. If you're filing an I-485, the answer is almost always yes.

Completion and Sealed Envelope

Dr. Ikyaator completes and signs Form I-693. You receive it in a sealed envelope, which you submit unopened with your application or bring to your USCIS interview. Do not open the envelope.

What USCIS Requires for Your I-693

Form I-693 identifies health conditions that could affect admissibility on public health grounds, and only a designated civil surgeon may perform it. The exam has four required parts: a medical history review, a physical examination, disease screening, and verification of your vaccinations. Each must meet current USCIS guidelines for your form to be accepted.

What the Physical Examination Covers

The civil surgeon examines your eyes, ears, nose, mouth, throat, neck, heart, lungs, abdomen, lymph nodes, skin and extremities, and assesses your mental health status through observation and conversation. You will also be asked about past and current conditions, surgeries, hospitalizations and ongoing treatment. Your medical history becomes part of your official immigration record, so accuracy matters.

Disease Screening by Age

Screening requirements depend on your age.

Tuberculosis – required for everyone aged two and older. Ages two to fourteen typically receive a skin test (TST) or IGRA blood test; ages fifteen and older generally require a chest X-ray, though an IGRA or TST may be used in some circumstances. If screening suggests possible infection, further testing may be needed before your I-693 can be completed.

Syphilis – a blood test is required at fifteen and older. A positive result requires documentation of appropriate treatment.

Gonorrhea – required at eighteen and older; the method depends on your circumstances and current USCIS guidance.

The civil surgeon also evaluates for other Class A conditions that would make an applicant inadmissible. If anything comes up, Dr. Ikyaator will talk you through the next steps.

Vaccination Requirements

USCIS requires proof of every vaccine recommended by the Advisory Committee on Immunization Practices for your age group. Depending on age and medical appropriateness, that can include mumps, measles, rubella, polio, tetanus and diphtheria toxoids, pertussis, haemophilus influenzae type B, hepatitis A, hepatitis B, rotavirus, meningococcal disease, varicella, pneumococcal disease and seasonal influenza. Not every vaccine applies to every applicant, and some may be waived when they are not medically appropriate.

If your records are incomplete you have options: receive the missing vaccines during your exam, get them elsewhere and return with documentation, or in some cases use blood tests to confirm immunity from prior vaccination or infection. We administer most required vaccines in office, which usually saves a second appointment. See the full vaccine guide.

Form Validity and Timing

The civil surgeon’s signature on Form I-693 is valid for two years. USCIS generally requires the form to be signed no more than 60 days before you file, and expects to decide your application within two years of that signature. Because the timing rules are easy to get wrong, we recommend scheduling your exam shortly before you plan to submit. If you are unsure, check with your immigration attorney.
Before your appointment

Come Prepared. Avoid Extra Visits.

Having the right documents at your appointment keeps things moving and reduces the chance of needing a follow-up visit.

  • ✓Any form of ID
  • ✓Your medical insurance card, if you have one — lab tests and vaccines are billed separately
  • ✓Your vaccination records — all available documentation
  • ✓Payment for the balance of your exam and any required vaccines

Vaccination records are the single most important item. Bring everything you have — childhood records from your home country, school immunization records, vaccination cards, and any US medical records showing vaccines received. Records in languages other than English are accepted, and partial documentation still reduces the number of vaccines you need.

Common Questions

The questions Houston applicants ask us most often.

How much does the immigration medical exam cost?+
The examination itself is $300, which covers your physical exam, completion of Form I-693, review of your medical history and vaccination records, and the documentation USCIS requires. Laboratory tests and any vaccines you need are billed separately, because they vary by age and circumstance. You pay a $50 deposit when you book and the balance at your visit, and we review the exact costs with you before any testing is done.
Does insurance cover it?+
Generally no. Immigration medical exams are treated as an immigration requirement rather than a medical necessity, so health insurance does not usually cover them. We do not bill insurance for these exams.
What if I do not have my vaccination records?+
Bring whatever you have. Partial records still reduce the number of vaccines you need. If records are missing entirely, you can receive the vaccines during your exam, obtain them elsewhere and return with documentation, or in some cases have titer blood tests to confirm immunity you already have. Titers are often cheaper than repeating a full vaccine series.
How long does the appointment take?+
Plan for about an hour for the examination itself. If you need vaccines or laboratory work, allow a little longer. Coming prepared with your documents is the single biggest factor in avoiding a second visit.
When will I get my results?+
Dr. Ikyaator completes Form I-693 and gives it to you in a sealed envelope for submission to USCIS. Do not open it - USCIS will reject a form that has been opened. If laboratory results are pending, we will let you know when your sealed envelope is ready.
When should I schedule my exam?+
Shortly before you file. The civil surgeon's signature is valid for two years, but USCIS generally requires the form to be signed no more than 60 days before you file your application. Scheduling too early can mean repeating the exam.
Do you see children?+
Yes. Requirements vary by age - tuberculosis screening begins at age two, syphilis testing at fifteen, and gonorrhea testing at eighteen - and the vaccine schedule is age-appropriate. Bring your child's immunization records.
Transparent pricing. No surprises.
$300the examination
$50deposit to book

The $300 covers your physical exam, completion of Form I-693, review of your medical history and vaccination records, and all documentation required for USCIS submission.

Laboratory tests — tuberculosis, syphilis and gonorrhea screening as required for your age — are billed separately, and we review the exact cost with you before any testing is performed. Vaccines and titer blood tests are also billed separately. If your vaccination records are complete you may need none at all, which is why bringing thorough documentation is the most reliable way to keep your total down.

You pay a $50 deposit when you book and the balance at your visit. The deposit holds your appointment and is applied to your exam. Immigration medical exams are generally not covered by health insurance, so we do not bill insurance for them. We accept cash, credit and debit cards.

Deposits are non-refundable. Appointments may be rescheduled once.
